    I use dvdcovergold - http://www.cheapshareware.co.uk/products/dvdcovergold/.
Best investment i ever made - allows for customised sizes, can adjust the spine, height, width of cover.
Its very easy to use and very flexible.
I save the covers I have manipulated with dvdcovergold as a PDF and then get them printed out at my local printshop - saves me a fortune in printer cartridges

I like to keep it simple. Drop the downloaded cover into Word, set the right size, Print. (Using 135Gm single sided glossy paper)

You could order the cases on line through Shop Cases, i find them ti be the cheapest, but do your shopping on line, I have been using these guys for years and i find them to be good quality and cheap. As for paper i use Brochure Paper Glossy 48 lbs. you get 150 per box, purchase them at office depot, they sell the box 2 for 1 on coming up holidays, you print comes out perfect and its much better paper, i find this to be cheap then to use the pre-sized ones that r super expensive. you will pay $28.oo per box, but around mothers day, easter, thanksgiving u get two for the one and i stock up.

I use DVD Print Master. It works reasonably well. However, I do use an HP printer so the end result can be sort of wonky without some fine tuning.
